Sushi Den Contest Rules & Eligibility

1. No purchase is necessary to win. Entry forms are also available at Sushi Den Restaurant. Only completed, original entry forms will be eligible. Winners will not be determined by their comments included on the entry forms. Entries may be submitted through Sushi Den website, at the restaurant, by mail or by fax. No limit on the number of entries per person.
2. One (1) $100 and five (5) $50.00 Sushi Den Gift Certificates will be awarded each month, in a drawing conducted by a member of Sushi Den management on the last day of each quarter. The Gift Certificates are redeemable only at Sushi Den Restaurant and will expire 12 months from the date of issue.
3. Winners will be notified by either e-mail or telephone and the Gift Certificates will be mailed to all winners.
4. Sushi Den assumes no responsibility for lost, mutilated, or misdirected entries.
5. All transportation costs and/or sales taxes are the responsibility of the winners.
6. Contest entrants must be 21 years of age, or older.
7. Employees and families of Sushi Den Restaurant are not eligible.
8. No prizes are redeemable for cash.
9. Winners agree to provide proof of eligibility, comply with contest rules, allow a post-winning interview, and allow the use of their names and or photographs in future promotions.
10. Odds of winning depend on the number of entrants received.
11. Entrants may request a list of Prize Winners by sending a stamped, self-addressed envelope to Sushi Den, Attn: Contest Winners, 1487 S. Pearl St., Denver, CO 80210.
12. Void where prohibited.
